My Parent's Concierge is a national coordinator of quality of life services. Using a national network of certified Geriatric Care Managers, Trust and Estate Counsels/Officers, Physicians/Pharmacists, Home Health Agencies, and Continuing Care Facilities, MPC can help the adult children manage the care of aging parents and relatives.

MPC can develop a Personalized Plan for each client with ongoing monitoring that includes:

  • A comprehensive on-site assessment of the beneficiary

  • The preliminary investigation of services

  • Developing a plan of recommended services that includes recommended vendors and providers

  • Revisions and updates of the plan as necessary or requested

MPC can investigate and assess what community resources are available to beneficiaries such as:

  • Activities of daily living: Potential providers are identified with referral recommendations; we can facilitate and/or conduct negotiations with selected provider.

  • Activities of socialization: We can investigate what volunteer or work opportunities might be available, how spiritual needs can be met, or what entertainment or group activities exist.

  • Alternative living arrangements: Those include continuing care communities, assisted living arrangements, nursing home options; group or congregate living options, in-technology applications, home support/caregiver options; and independent living options.

Medication Managment

Too often, people are discharged from hospitals or leave the doctor's office with unanswered questions. Many of those questions are about which medications to take or resume taking, when to take it, what dosage is correct or what precautions to take.

My Parent's Concierge offers a special medication management service, "My Personal Concierge/mm," to address these matters. Certified clinical pharmacists and nurses work with you, in your home, to review all medications you take. They identify potential concerns, such as drug interactions or duplications.

MPC/mm expedites coordination between the clinical pharmacists, physicians, your local pharmacists, and, if you wish, your family. My Personal Concierge/MM provides:

  • A personal concierge as your contact person for guidance, questions or concerns regarding your medication

  • Review of all medications you take in your home

  • Assistance in obtaining and renewing medications

  • Phone contact from your personal concierge at least every two weeks

  • An easy to carry medication record containing their names and dosages

A Note On Fees

Every client receives personalized services. My Parent's Concierge assigns a personal concierge to each client at no charge. Fees vary depending on the type of services needed, level of case management required and all fees are discussed and agreed upon with our clients before any work is started. My Parent's Concierge fees are incorporated in our membership fees, and hourly case management fees (where applicable). My Parent's Concierge accepts no referral fees from vendors and therefore is only motivated to provide the client with the best service options.
